The On-Boarding Process
Complete the Application, Permission to Request Background Check*, and Photo Release forms online or visit one of our service centers to complete a hard copy.​

A CCS representative will typically call you within 3-5 business days to discuss available opportunities and options.
* To ensure the safety of our clients, staff, and volunteers, all volunteer applicants are required to provide their full legal name, date of birth, government-issued identification information, Social Security number, and signature as part of the application and background screening process. All personal information is kept strictly confidential, used solely for volunteer screening and recordkeeping purposes, and is never sold, shared, or released to third parties except as required to complete authorized background checks or by law.
